| 3701-31-04 (B)(6)(f) Required turnover rates were not met. Flow was at 60 gpm. Minimum flow required is 75gpm. Operator is working on increasing the flow. Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.  |
