3701-31-04 (B)(1)(a)
Critical Outlet covers were not secured.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation.
3701-31-04 (B)(4)(a)(i)
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Repeat Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Repeat pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.

pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range.
Comments
Establishment intends on acquiring an automatic chemical controller (ACC). An ERN (Equipment Replacement Notification) form DOES NOT NEED TO BE SUBMITTED TO ADD A CONTROLLER. The only requirement to add an ACC is that it is on the approved list of controllers from ODH (provided at time of inspection). An ERN is only required when a piece of equipment must be replaced with a different make and model from what was originally approved for the licensed pool. If the piece of equipment is being replaced with an exact replica, then no ERN is required.
