| 3701-31-04 (B)(6)(f) Required turnover rates were not met. The flow was measured at 150gpm at time of inspection and needs to be above 2128 gpm. Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.  |

| The flow meter is reading 150 gpm. The minimum required flow is 218 gpm. Adjust equipment or repair flow meter to ensure the flow is at 218 gpm or greater. New disinfection feeder and acid feeder are do not have the same model number as the previous feeders. Submit Equipment Replacement Notification for new disinfection and acid feeders. A follow up will be conducted on Oct 21, 2016 to observe correct flow rate. |
