3701-31-04 (A) Repeat Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). Submit ERN form for new pump/motor. Contact ODH to obtain approval for any construction/alterations before operating pool.

3701-31-04 (B)(6)(i) Repeat A means of determining rate-of-flow was not found for all water pumps. Pump curve was not available at time of inspection. Properly install a flow meter or have a pump curve available for every water pump used for the pool.

The following violation(s) have been corrected since the last inspection.
3701-31-04 (C)(2)/pH is within 7.2 to 7.8 pH was outside the range of 7.2 to 7.8. pH was at 7.0 Treat the pool water so that the pH is within the acceptable range.
3701-31-04 (E)(2)(d)/First aid kit provided First aid equipment was not provided. Provide first aid equipment, including a first aid kit, at the pool at all times.
