Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. (AT TIME OF INSPECTION THE LADDER WAS OBSERVED TO BE LOOSE AND SEVERAL OF THE SLIP RESISTENT PADS ARE MISSING COMPLETELY)
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
