| 3701-31-04 (B)(6)(f) Required turnover rates were not met. The flow needs to be at least 413 gpm. Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. |
| 3701-31-04 (B)(6)(o) Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. The two ladders near the diving boards are loose. Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. |
