Repeat Cross connection hazards were present (AT TIME OF INSPECTION THE FILL HOSE WAS BEING FED DIRECTLY INTO POOL FILL OUTLET. REMOVE THIS CROSS CONNECTION)
Remove all cross connection hazards from the pool area.
3701-31-04 (B)(6)(f)
Required turnover rates were not met. (AT TIME OF INSPECTION THE FLOW RATE WAS 0 GPM WHICH WOULD NOT MEET THE REQUIRED TURNOVER RATE)
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (E)(2)(a)(ii)
A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. (AT TIME OF INSPECTION THE NEW FLOTATION DEVICE WAS OBSERVED TO NOT BE USCG TYPE IV NOR WAS THE ROPE ATTACHED)
Provide a proper floatation device at all times.
3701-31-04 (E)(3)(b)
The unguarded pool was in use without the "Children must be supervised" and "Swimming alone is not recommended" signs posted. (AT TIME OF INSPECTION THERE WAS NO "SWIMMING ALONE IS NOT RECOMMENDED" SIGN POSTED)
Post the "Children must be supervised" and/or "Swimming Alone is Not Recommended" signs at all times the pool is available for use.
