3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. Guard chairs have broken bases. Manager stated that new bases have been ordered. All equipment must be maintained in accordance with the manufacturers specifications.

3701-31-04 (B)(6)(o) Repeat Ladders/stairs/steps were not in good repair or were not slip resistant. Ladders in the dive well are loose. Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.

Maintenance gate is being used for egress to promote directional traffic. Normally, maintenance gate needs to be kept locked. Make sure to test flow switch monthly and document. Tested at time of inspection.
