3701-31-04 (B)(6) Equipment was not maintained per manufacturers specifications. The deck drain cover near the equipment room has a broken section. All equipment must be maintained in accordance with the manufacturers specifications.

3701-31-04 (E)(2)(e) Emergency device (telephone) was not working. Phone was present, however did not have a dial tone. Ensure that the emergency device is functioning properly and rings to the emergency response authorities.

Operator has a work order in for repairing the emergency phone and is expected to be done today. A follow up inspection will be conducted in the afternoon on 6/1 to observe the emergency phone working.
