3701-31-04 (A) Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). Circulation pump has been replaced within the last year with a different model than the Challenger pump that was originally approved. ERN left at time of inspection. Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).

3701-31-04 (B)(4)(a)(ii) Combined chlorine was not tested daily at pool side prior to use. Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use.

Pool was closed at time of inspection due to chair lift being broken. pH, Free Chlorine, Total Chlorine, need to be tested and recorded prior to opening and every 12 hours while open. Total Alkalinity needs to be tested and recorded weekly. Flow switch needs to be tested and recorded monthly.
