| 3701-31-04 (B)(4) The licensee /authorized representative was not keeping or maintaining operational records. At time of inspection, water quality (Free Chlorine, Combined Chlorine, PH, Water Temperature) were not recorded for several days while in operation. These items need to be tested prior to bather entry and at least every 12 hours while open for swimming. Record these items on the records for the spa. All facility records must be maintained and available for review upon request.  |

| Pressure switch was tested at time of inspection. Make sure this is tested and recorded monthly. Jet pump has been replaced with a new pump that is a different model. Submit Equipment Replacement Notification form to Ohio Department of Health. |
