3701-31-04 (A)
Repeat Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). (THE DECKING HAS BEEN REPLACED/ALTERED WITHOUT OBTAINING APPROVAL FROM ODH)
Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (B)(4)(a)(i)
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features.
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features.
pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.

Ladders/stairs/steps were not in good repair or were not slip resistant. (AT TIME OF INSPECTION ONE OF THE LADDERS WAS LOOSE)
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(q)
Repeat Depth markers/deck warning signs were not maintained in a clearly legible/visible manner.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated.
Comments
Pool chemistry MUST be tested at the required intervals EVERY DAY THAT THE POOL IS OPEN. There must be a plan in place on who will take care of the pool and perform the required testing when the main operator is unavailable. A reminder that the pool chemistry must be tested every day prior to the pool opening/bathers enter the water and then every 4 hours that the pool is open/operational.
