3701-31-04 (A)
Pump intake for the water slide has been moved from the approved location in the overflow sump pit to directly from the pool without approved plans. This is not an appropriate setup and may be a danger to patrons.
Contact Ohio Department of Health Engineering at 614-466-1390 for plan submission or move the intake to approved location.
3701-31-04 (B)(1)(a)
Critical Outlet covers were not compliant covers. The intake from the water slide has been moved to directly out of the pool and does not have approved intake covers. Location and alterations have not been approved by Ohio Department of Health.
Do not operate the pool without compliant outlet covers. Install compliant outlet covers and obtain approval from Ohio Department of Health prior to placing the pool back into operation.
3701-31-04 (B)(4)(a)(ii)
Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Water temperatures were not being recorded daily.
Take and record the water temperature at least once/day.
Comments
Submit plans for changes to water slide or move water intake to approved location. Pump intake does not have approved covers for intake and could pose an entrapment/entanglement issue. Pool should not be operating without approved covers, water slide needs to taken out of service until alterations are approved or pump intake is moved back to the approved location. Slide was voluntarily closed and shut down at time of inspection. Make sure to test and record flow switch at least monthly. Flow switch was tested at time of inspection and appears to be operating appropriately.
