3701-31-04 (A)
Repeat Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN). (THE HAYWARD PUMP THAT IS CURRENTLY INSTALLED DOES NOT HAVE ODH APPROVAL AND AN EQUIPMENT REPLACEMENT NOTIFICATION FORM MUST BE SUBMITTED. AND ERN WAS PROVIDED)
Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool.

Corrected During Inspection Pool water temperature was above 90 degrees Fahrenheit without approval from the Director of Health. At time of inspection, the pool temperature was at 93F. Maintenance turned off the heater and added cold water to the pool to bring the temperature of the pool to 90 degrees.
Either reduce the water temperature of the pool to below 90 degress Fahrenheit or seek approval to operate at a higher temperature from the Director of Health.
The following violation(s) have been corrected since the last inspection.
3701-31-04 (B)(1)(c)/Minimum disinfection residual shall be maintained as required.
Chlorine disinfectant residual was less than 1 ppm for the pool (AT TIME OF INSPECTION FREE CHLORINE WAS AT .5 ppm)
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation.
3701-31-04 (B)(4)(a)(i)/Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. (AT TIME OF INSPECTION IT WAS OBSERVED THAT NO POOL CHEMISTRY IS NOT BEING PERFORMED AND DOCUMENTED. POOL CHEMISTRY MUST BE CONDUCTED EVERY DAY, PRIOR TO BATHERS ENTERING THE POOL AND EVERY 12 HOURS THAT THE POOL IS OPEN TO BATHERS.)
Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use. If an ACC is in use the disinfectant residual must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the disinfectant residual must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Emergency device (telephone) was not working.
Ensure that the emergency device is functioning properly and rings to the emergency response authorities.

SVRS/APSOS/ACC test results/ service records were not maintained.
The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.
Comments
Flow switch for the controller was tested at time of inspection. By telephone, manager John Lieb described how to test the SVRS. These two items need to be tested and results recorded on a monthly basis. Manager Katy Carroll is working to complete the ERN for the pump that has been replaced. Free chlorine is now above 1.0 ppm, daily testing is now being completed before bathers enter the pool and at least every 12 hours, backwash line is now air gapped, new reagents were purchased, flotation device has been corrected, emergency phone was tested. Pool is ok to reopen. Make sure to test the pool water temperature at the pool deck.


Inspection Outcome:
