| 3701-31-04(D)(6)(a)(2) Critical Pool free chlorine disinfectant residual was below 1 ppm. Free chlorine was at .6 ppm. Pool was just opening at time of inspection and hadn't been tested by the operator yet. Controller was adjusted and free chlorine has gone above 1.0 ppm. Do not operate the pool until the chlorine disinfectant residual is at 1 ppm or above.  |

| Chlorine feeder has been changed to a Stenner 45M5 and is feeding sodium hypochlorite. Submit Equipment Replacement Notification form to Ohio Department of Health with payment. Emergency phone tested at time of inspection. Flow switch was tested at time of inspection. Make sure to test and record flow switch on a monthly basis. |
