[?] A summary of the violations found during the inspection are listed below.

3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
Repeat ORP reading was below 650 mv. It was observed on ACC at between 640-645 mv reading.
Have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Observed pH feeder not replaced like for like. Stenner 45M5 was replaced with Stenner 85MHP5. Provided EARN form to submit to ODH on-site. Discussed alternative as well (replace again with like for like).

3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Observed loose/damaged faucet for women's restroom handsink. No hot water was obtained at this sink.

3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Main drain paperwork was unavailable at time of inspection. Operator noted the main drains have just been replaced. A recheck will occur.
Make available main drain paperwork stating when install date was and how long main drain is good for until needing replaced.
3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Combined chlorine was observed not tested daily at pool side prior to use(not recorded on log).
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use.
3701-31-04 (B)(4)(a)(iv) / Water temperatures are recorded daily
Water temperatures observed not being recorded daily on log.
Take and record the water temperature at least once/day.
3701-31-04 (B)(4)(b) / Life guard/ CPR/First Aid training certificates are current and available
Life guard/ CPR/First Aid training certificates were not current for two lifeguards. They expired in February of 2023.
Provide documentation of all training certificates for all lifeguards.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met as minimum flow rate was not reached per flow meter. Flow meter reading was 130gpm and minimum flow rate to achieve required turnover rate is 250gpm.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat Observed faded/missing patches of non-slip grip on kid's stairs, with paint also peeling.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Observed gate leading from wading pool area to main pool area not self-closing at either 45 degrees or 90 degrees.
Repair or replace gate with self-closing gate.
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area. Operator noted this is in progress, and signs will be posted by next week.
