3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Tempered water was not available at men's and women's restroom hand washing sinks.

3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Combined chlorine was not tested daily at pool side prior to use.
Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use. If ACC is in use combined chlorine must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the combined chlorine must be tested at 1 spray feature every 6 hours the sprayground or spray feature is in use.

Water temperatures were not being recorded daily.
Take and record the water temperature at least once/day.
3701-31-04 (B)(4)(a)(vii) / Cyanuric acid is tested weekly
Cyanuric acid was not tested weekly.

Main drain paperwork was not available during today's inspection. Make and model numbers of main drains, and date of installation are needed.
Maintain all records and have them available upon request.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Flow rate was observed at 970 gpm, but the minimum flow rate is 1042 gpm.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Multiple entry ladders on pool deck were observed loose.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(t) / Equipment/chemical/storage areas are free of hazards/accessible only to authorized personnel.
Critical The slide pump area was enclosed in an area with a gate, but it was not locked to prohibit access to this area. Action plan discussed on-site, and a pad-lock will be installed to gate.

3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm, as it was tested at 50 ppm.
Raise the total alkalinity to at least 60 ppm.
3701-31-04 (E)(4)(b) / Written plan for lifeguard coverage available for all pools 6,000 sq. ft or larger
The written lifeguard coverage plan was not available for review for pools 6,000 sq. ft or larger.
Prepare and provide a written lifeguard coverage plan that contains all the required elements according to the rule. The plan must be available for review upon request.
