[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Repeat Observed loose/damaged faucet for women's restroom hand sink. No hot water was obtained at this sink. A burnt out light above toilet stall in men's restroom was observed. Pool operator noted a work order is in to repair light.
Repair faucet and light.
3701-31-04 (B)(4)(a)(vii) / Cyanuric acid is tested weekly
Cyanuric acid observed not tested weekly or recorded on logs.
Test for and record cyanuric acid level at least once/week.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met as minimum flow rate was not reached per flowmeter. Flow meter reading was 180gpm and minimum flowrate to achieve required turnover rate is 250gpm.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat Observed faded/ missing patches of non-slip grip on kid's stairs, with paint also peeling.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Observed gate leading from wading pool area to main pool area not self-closing at either 45 or 90 degrees.
Repair or replace with self-closing gate.
