3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Repeat Observed concrete pulling away from the pool edge on the shallow side of the pool. Observed concrete cracking and deterioration surrounding the ladder by the lifeguard stand at the 12ft side of the pool. Observed peeling of surfacing material on the diving board. Observed deterioration of concrete wall behind the diving boards.

3701-31-04 (B)(4)(a)(vi) / Total alkalinity is tested weekly
Total alkalinity was not tested weekly.
Discussed with operator to ensure Alkalinity is being tested and recorded weekly.
3701-31-04 (E)(2)(b)(i) / 12 ft. non-telescopic reach pole with Sheppard's crook & a USCG Type IV personal flotation device with 30 - 60 foot throw line.
Repeat 12 ft. non-telescopic reach pole with Shepherd's crook was not provided at the guarded pool.

3701-31-04 (E)(4)(b) / Written plan for lifeguard coverage available for all pools 6,000 sq. ft or larger
Repeat The written lifeguard coverage plan was not available for review for pools 6,000 sq. ft or larger.
After discussion with operator, written plan for adequate lifeguard coverage will need to include a diagram indicating location of lifeguard station and coverage.
