Repeat Observed deteriorated surfacing on diving boards. Observed general concrete repairs needed at pool deck, especially along the perimeter of the gutters in the shallow end. Observed some build up/staining on pool floor in shallow end.

Repeat Observed loose pool deck ladder at 12 ft end.

3701-31-04 (E)(2)(a)(i) / 12 ft. non-telescopic reach pole with shepherd's crook is provided
Repeat The shepherd's crook must be 12 foot in length and non-telescopic.

3701-31-04 (E)(4)(e)(viii) / Lifeguards are currently certified by an approved entity for life guard training
Repeat One of the life guards on staff has a lifeguard/CPR/first aid certification that is not approved by ODH. Operator to contact OHH to confirm if the certifications are valid in the State of Ohio.
