[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B) / All facilities are maintained clean, safe and sanitary condition and in good repair.
Repeat Observed missing cover on return outlet in the bottom of the pool. A new cover will need to be purchased and placed on the drain.

3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat New drain cover was installed prior to the pool opening but the facility did not have the required paperwork on site. Operator stated that the paperwork would be brought on site.

3701-31-04 (B)(1)(e) / Automatic Chemical Controller (ACC) shall function properly.
Critical Automatic Chemical Controller (ACC) was not functioning properly at the time of the inspection. Ensure that the ACC unit is functioning properly prior to placing the pool back into operation. Facility is currently hand dosing the pool at night and maintaining correct levels of disinfection. A service call was already placed and a new one is set to be installed this week.

3701-31-04 (B)(4)(a)(iv) / Water temperatures are recorded daily
Water temperatures were not being recorded daily. Take and record the water temperature at least once/day

3701-31-04 (B)(4)(a)(vii) / Cyanuric acid is tested weekly
Cyanuric acid was not tested weekly. Test for cyanuric acid level at least once/week.

3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met, the gpm was observed at 59 gpm instead of the required 61. Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Repeat All ladders/hand rails were observed loose. Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Repeat Gate observed unable to self-latch when released. Ensure gate can self-latch to prevent unauthorized entry.

3701-31-04 (E)(2) / The public swimming pool has safety equipment visible
The spine board was not present in the pool area. Place the safety equipment in a clearly visible area and ensure that the equipment is accessible in the event of an emergency.

3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area. Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area.

Would recommend moving the sign for the first aid kit to a more highly visible area within the pool deck.
ORP reading unable to be taken since the ACC is non-functional. A new one is already on order and the facility is hand-dosing the pool and maintaining disinfection level. A recheck will be conducted this week once scheduled repairs have been made.
Facility is currently hand-dosing with Tri-Chlor.
