Repeat Required turnover rates were not met. Observed the flow meter at 175 gpm, the required minimum flow is 250 gpm for this facility.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

Safety line(s) were not maintained in place. The safety line was not present during time of inspection because the hook that connects the line was broken.
Safety lines must be in place when required to alert the bathers of the change in depth of the pool.

A USCG Type IV personal flotation device with 30 -60 foot throw line was not provided at the unguarded pool. Observed no ring buoy with throw line present at time of inspection.
Provide a proper floatation device at all times.
