The licensee /authorized representative was not keeping or maintaining operational records. Observed records with no dates on them, unable to determine most recent weeks' records apart from previous weeks.
All facility records must be maintained and available for review upon request. Please ensure all records have dates marked for reference.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. Observed flow meter not reading flow/stuck at bottom of meter.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat pH was outside the range of 7.2 to 7.8. Observed pH at 7.0
Treat the pool water so that the pH is within the acceptable range.
