[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(1)(b) / SVRS device(s) shall be functional.
Critical The SVRS device that is being used as a secondary means to prevent entrapment/evisceration was not functional at the time of the inspection. At time of inspection the SVRS system was not on and would not turn on.
Do not operate the pool when the SVRS is not functioning. Ensure the SVRS is operating properly prior to placing the pool back into operation. Correct By: 08-Jun-2018
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. The minimum required flow rate is 250 gpm, it was observed via the flow meter the flow at 175 gpm.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

6/8/18 - per discussion with pool operator and Chad Brown, the pool will be evaluated if an SVRS is required for the facility, if not required the system will be removed and violation will be removed.
