[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH). Operator has installed a new pump (Century Centurion Serial: 01219CH, 2.6 HP) to the system. Original pump was a Pentair WFE-8 with 2.0 HP.
Complete attached ERN form and send to ODH. Contact ODH to obtain approval for any construction/alterations before operating pool.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Ladders/stairs/steps were not in good repair. Observed loose handrail at steps leading into shallow end of pool.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8. At time of inspection, a pH of approximately 8.0 was measured at the pool.
Treat the pool water so that the pH is within the acceptable range.

Observed gap in perimeter gutter system at entrance of pool.
Documentation (make/model/year) on main drain cover is needed.
Since last inspection, operator has installed a new pump to the system. See updated equipment inventory form for specifications.
