3701-31-04 (B)(1) / Public swimming pool shall operate free of an imminent health hazard.
Critical Public swimming pool was in operation while an imminent health hazard existed
Do not operate the pool when an imminent health hazard is present. Fix the imminent health hazard before the pool is put back into operation. INCREASE FREE CHLORINE LEVEL TO AT LEAST 2 PPM. CLOSE SPA UNTIL CORRECTED. Correct By: 13-Oct-2021
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not secured.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE MISSING SCREW IN THE MAIN DRAIN COVER.
3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Chlorine disinfectant residual was less than 2 ppm for the spa
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 2 ppm or above prior to placing the spa back into operation.

3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.
ORP reading was below 650 mv
Have the ACC unit inspected to determined why the ORP is low.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining operational records.
All facility records must be maintained and available for review upon request. KEEP A LOG SHEET OF DAILY WATERAND OTHER TESTS
3701-31-04 (D)(8) / Acceptable test kit provided/adequate/in good repair
The test kit provided was not in good repair.
Maintain the test kit in good repair to conduct water quality tests. OBTAIN NEW TEST KIT
