3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Chlorine disinfectant residual was less than 1 ppm for the pool
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. THIS VIOLATION IS A CRITICAL OPERATIONAL ITEM AND POSES AN IMMINENT HEALTH HAZARD. CLOSE THE POOL UNTIL CORRECTED. THE OPERATOR CLOSED THE POOL AND IMMEDIATELY STARTED TO CORRECT THE PROBLEM.

3701-31-04 ( D )(6)(a) / ORP reading shall be 650 millivolts or greater.

Have the ACC unit inspected to determined why the ORP is low. INCREASE ORP Correct By: 12-Aug-2021

Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired.
