3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly -Observed the ACC was not recorded as being tested in the month of June. -Test the ACC monthly and recorded when it was performed.

3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met. -Observed the flow rate of 100 was below the required minimum flow of 108 to achieve the proper turnover rate of 480 minutes. -Discussed possible reasons the flow meter is reading so low.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.

If moving the flow meter, it should be installed on a straight length of pipe at a distance of at least 10 pipe diameters downstream and 5 pipe diameters upstream from any valve, elbow, or other source of turbulence. For example: if you have 2 inch piping, your flow gauge should be installed 20 inches downstream and 10 inches upstream from any source of turbulence.
