Critical Outlet covers were not installed properly. hole drilled into drain cover. Reportedly the frame is broken.
Do not operate the pool when the outlet covers are not installed. INSTALL SUCTION OUTLET FITTING ASSEMBLY PROPERLY. REPLACE THE DRAIN COVER FRAME IF IT IS CRACKED OR BROKEN. INSTALL A NEW COVER ACCORDING TO MANUFACTURER'S INSTRUCTIONS. DO NOT DRILL HOLES IN THE COVER.

3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat SVRS and ACC test results were not maintained.
The test results for these devices must be kept on site for review. TEST AND RECORD SVRS AND ACC RESULTS MONTHLY.
3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Repeat Required meters and gauges were not maintained or were not functional while the pool is in operation.
Maintain all meters and gauges in a properly functioning manner at all times while the pool is in operation. REPAIR OR REPLACE FLOW METER. IF REPLACED, RESIZE THE FLOW METER IN THE RANGE CLOSER TO THE POOL FLOW (20-120 GMP).

Repeat First aid equipment was not provided.
Provide first aid equipment, including a first aid kit, at the pool at all times. PROVIDE A FIRST AID KIT IN POOL AREA.

The operator was observed performing onsite chemical tests appropriately and is aware of the acceptable parameters and necessary corrective actions.
