Repeat Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).

3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE THE FLOW RATE TO 69-120 GPM
3701-31-04 (B)(6)(i) / A means of determining rate-of-flow on all water pumps
Repeat A means of determining rate-of-flow was not found for all water pumps.
Properly install a flow meter or have a pump curve available for every water pump used for the pool. INSTALL PRESSURE AND VACUUM GAUGES, POST PUMP CURVE AND CALCULATION FOR DETERMINING THE TOTAL FEET OF HEAD.

Repeat First aid equipment was not provided.
Provide first aid equipment, including a first aid kit, at the pool at all times. PLACE FIRST AID KIT IN POOL AREA.

This inspection is made with the Environmental Health Supervisor, Greg Putka, because the time frame has passed for correction of previously cited violations. A meeting will occur with the pool company that cares for the pool in the near future to go over outstanding violations
