Throttle valve was not properly maintained/adjusted. The throttle valve was not tagged.
TAG THE THROTTLE VALVE WITH THE MAX ALLOWABLE FLOW AND THE MIN REQUIRED FLOW OR PLACE ON THE PIPE BY THE FLOW METER. MAX FLOW IS 566 GPM AND THE MIN IS 314 GPM. Correct By: 16-Jun-2017

Depth markers/deck warning signs were not maintained in a clearly legible/visible manner. Depth markings were more than 25 ft apart.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. DEPTH MARKINGS AND "NO DIVING" MARKINGS MUST BE NO MORE THAN 25 FT APART. ADD DEPTH AND "NO DIVING" MARKINGS WHERE THEY ARE MORE THAN 25 FT APART Correct By: 10-Jul-2017

Critical Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. The flow switch/pump interlock was not tested and recorded monthly.
