3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Public swimming pool was constructed, installed, or substantially altered without obtaining approval from the Ohio Department of Health (ODH).
Contact ODH to obtain approval for any construction/alterations before operating pool. SUBMIT AN EQUIPMENT REPLACEMENT NOTIFICATION FOR THE REPLACEMENT OF THE PUMP AND THE CHLORINATOR. THE HAYWARD SP2615X20 1.5 HP WAS REPLACED WITH A HAYWARD SP2607X7 .75 HP PUMP. THE RAINBOW 300-29X EROSION FEEDER WAS REPLACED WITH A STENNER 45M5 PUMP FEEDER. Correct By: 11-Aug-2020

Deck surfaces had standing water or other hazards. The pool cover anchor is sticking up from the deck about a 1/2 inch.
