[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (B)(4)(a)(i) / Disinfection residuals are tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat Disinfection residuals were not tested daily at pool side and/or at the required intervals for pools/automatic chemical controllers/special features. - Observed the testing of pool chemicals is not being documented as tested every 4 hours of being open. -Test the disinfection residual every day before bathers enter the water. Disinfection residual must then be tested every 4 hours when an ACC is not in use.

3701-31-04 (B)(4)(a)(ii) / Combined chlorine is tested daily at pool side prior to use
Repeat Combined chlorine was not tested daily at pool side prior to use. - Observed the combined chlorine is not being recorded every day the pool is open or every 4 hours the pol is open as required. -Test for combined chlorine prior to bathers entering the water and every 4 hours the pool is open for use.

3701-31-04 (B)(4)(a)(iii) / pH is tested daily at pool side at required intervals for pools/automatic chemical controllers/special features
Repeat pH was not tested daily at pool side and/or at required intervals for pools/automatic chemical controllers/special features. - Observed the pH is not being recorded every day the pool is open or every 4 hours the pool is open as required. -pH must be tested prior to bathers entering the water and every 4 hours the pool is open for use.
If ACC is in use pH must be tested every 12 hours while the pool is in use. For spraygrounds and spray features, the pH must be tested at 1 special feature every 6 hours the sprayground or spray feature is in use.
3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat SVRS/APSOS/ACC test results/ service records were not maintained. - Observed no documentation of testing or knowledge of how to test SVRS system. -The test results/service records for these devices must be kept on site for review. If the device has not been recently serviced, have the device tested immediately and provide documentation of the test results.

3701-31-04 (B)(6)(h) / Required meters and gauges maintained/functional
Required meters and gauges were not maintained or were not functional while the pool is in operation. -Observed flow meter is broken and not functional. -Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation.

3701-31-04 (B)(6)(j) / All SVRS devices are maintained/ functional and tested
Critical SVRS device was not tested in accordance with the manufacturer's specifications. -Have the SVRS device tested immediately and maintain the test results on site.

3701-31-04 (B)(6)(p) / Deck surfaces are free of standing water or other hazards.
Repeat Deck surfaces had standing water or other hazards. -Observed areas of the deck are cracked and create a tripping hazard as well as holding standing water. Repair areas of the deck where standing water is occuring. If more than 50% of the deck must be replaced, plans must be submitted to the Ohio Department of Health for review and approval.

3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm. - Observed the alkalinity documented was consistently below 60 ppm. - Raise the total alkalinity to at least 60 ppm.
