3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Outlet covers were not secured. Two screws on each drain cover are missing.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE MISSING SCREWS IN THE DRAIN COVERS.

Critical Repeat Outlet covers were not compliant covers. Equalizer outlets are not plugged
Do not operate the pool without compliant outlet covers. Install compliant outlet covers prior to placing the pool back into operation. PLACE COMPLIANT COVERS OVER THE SKIMMER EQUALIZER OUTLETS.
3701-31-04 (D)(8)(c) / Test kit reagents fresh/ properly stored.
Test kit reagents were not fresh or stored properly.
Obtain fresh reagents and store them properly. REPLACE EXPIRED REAGENTS. Correct By: 14-Jun-2022

HAVE THE BACKFLOW PREVENTER INSPECTED ANNUALY AND KEEP DOCUMENTATION AVAILABLE AT THE POOL.

Personal Protective Equipment (PPE), chemical gloves and goggles, are available for handling chemicals. Keep the chemicals’ Safety Data Sheets (SDS) in the equipment room

The Certificate of Compliance for the main drain cover(s) is/are available in the equipment room. Our records show that the main drain suction outlet covesr (Waterway 640-472xV) were installed on 5/22/2019. The covers have a 7 year life span and must be replaced before 5/22/2026. This drain cover has a maximum flow rating of 356 gpm. When replacing the covers, ensure that the new covers' flow rating will not be exceeded. 360 gpm.


The operator was observed performing onsite chemical tests appropriately and is aware of the acceptable parameters and necessary corrective actions.
