3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).
Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool. APPROVAL FOR THE NEW CHLORINATOR HAS NOT BEEN RECEIVED BY THE LOCAL HEALTH DEPARTMENT. REPORTEDLY THE OPERATOR HAS SENT REQUEST TO ODH FOR CHLORINATOR APPROVAL.
3701-31-04 (B)(1)(c) / Minimum disinfection residual shall be maintained as required.
Critical Chlorine disinfectant residual was less than 1 ppm for the pool
Do not operate the pool when the disinfectant residuals are less than what is required. Ensure the chlorine disinfectant residual is at 1 ppm or above prior to placing the pool back into operation. THIS IS A CRITICAL OPERATIONAL ITEM AND POSES AN IMMINENT HEALTH HAZARD. OPERATOR CLOSED THE POOL AT THE TIME OF INSPECTION AND POSTED A SIGN ON THE DOOR TO THE POOL THAT THE POOL WAS CLOSED. KEEP POOL CLOSED UNTIL FREE CHLORINE LEVEL IS AT LEAST 1 PPM.
3701-31-04 (B)(4) / The licensee /authorized representative is keeping records
The licensee /authorized representative was not keeping or maintaining operational records.
All facility records must be maintained and available for review upon request. TEST AND RECORD WATER BEFORE OPENING THE POOL EVERY DAY. IF THE POOL WILL BE OPEN MORE THAN 12 HOURS THE CHLORINE AND pH MUST BE TESTED TWICE A DAY. Correct By: 13-Apr-2023
3701-31-04 (B)(4)(a)(vi) / Total alkalinity is tested weekly
Repeat Total alkalinity was not tested weekly.

Critical Chemical containers were stored without labels identifying the contents.
