Critical Repeat Outlet covers were not installed properly. Hole drilled into cover. Reportedly frame is broken.
Do not operate the pool when the outlet covers are not installed. INSTALL SUCTION OUTLET FITTING ASSEMBLY PROPERLY. REPLACE DRAIN COVER AND FRAME. DO NOT DRILL HOLES IN COVER.

Repeat Required meters and gauges were not maintained or were not functional while the pool is in operation.
Maintain all meters and gauges in a properly funtioning manner at all times while the pool is in operation. REPAIR FLOW METER.
