Cross connection hazards were present. The hose bib below the eye wash station in the filter room is not backflow protected.
Remove all cross connection hazards from the pool area. INSTALL AN ASSE 1011 BACKFLOW PREVENTER ON THE HOSE BIB BELOW THE EYE WASH STATION IN THE FILTER ROOM. Correct By: 09-Apr-2018
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range. ADJUST THE pH TO BETWEEN 7.2 AND 7.8. Correct By: 07-Apr-2018
3701-31-04(D)(6)(a)(1) / Combined chlorine shall not exceed 1.0 ppm.
Combined chlorine was greater than 1.0 ppm.
Treat the pool so that the combined chlorine is less than 1 ppm. SHOCK CHLORINATE THE POOL TO REDUCE THE COMBINE CHLORINE. MAX LEVEL IS 1.0 PPM COMBINE CHLORINE. Correct By: 09-Apr-2018

The operator was observed performing onsite Automatic Chemical Controller flow switch testing appropriately.

The main drain suction outlet cover (AVSCO A+A Manufacturer) was installed 5-23-2017. The cover has a 7 year life span and must be replaced by 5-23-2024.
