| Equipment requiring ODH plan approval has been replaced since the last inventory report dated 7-26-16. The disinfection feeder has been changed from a Flexflo A1N30A-7T to a Flexflo A1N10A-7T. Since the disinfectant feeder has a different capacity approval is required. Complete and submit a Equipment Replacement Notification form and submit to the Ohio Department of Health. |
