Equipment was not maintained per manufacturers specifications. A skimmer weir was missing in the skimmer next to the guard chair.
All equipment must be maintained in accordance with the manufacturers specifications. REPLACE THE MISSING WEIR IN THE SKIMMER LOCATED NEXT TO THE GUARD CHAIR. Correct By: 17-Jun-2020
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.
Cross connection hazards were present. The hose bib in the filter room did not have a ASSE 1011 backflow preventer.
Remove all cross connection hazards from the pool area. INSTALL AN ASSE 1011 BACKFLOW PREVENTER ON THE HOSE BIB IN THE FILTER ROOM. Correct By: 17-Jun-2020

Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE THE FLOW RATE TO AT LEAST 175 GPM.

Critical Chemical containers were stored without labels identifying the contents.
Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. LABEL THE CHLORINE TANK. Correct By: 17-Jun-2020
3701-31-04 (E)(3)(c) / Location of emergency telephone posted when the phone is not visible.
Sign indicating the location of the emergency device (telephone) was not provided and emergency device was not visible in the pool area.
Ensure that a sign informing patrons of the location of the emergency device is always visible in the pool area. POST A SIGN IN THE POOL AREA STATING WHERE THE EMERGENCY PHONE IS LOCATED. DO NOT KEEP PHONE IN A LOCKED ROOM.

Keep the chemical SDS (MSDS) in the equipment room and have available the PPE for handling chemicals.

Our records show that the main drain suction outlet cover (AquaStar WAV 12) was installed 8/7/2019. The cover has a 5 year life span and must be replaced before 8/7/2024.
The Certificate of Compliance was available in the guard room.
