Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE THE FLOW RATE TO AT LEAST 1208 GPM. Correct By: 22-Jun-2021

Comments
The operator was observed performing onsite chemical tests appropriately and is aware of the acceptable parameters and necessary corrective actions.

The Certificate of Compliance for the main drain cover is available in the office.
Our records show that the main drain suction outlet covers (AquaStar WAV 12 101) were installed 5-14-21. The covers have a 5 year life span and must be replaced before 5-14-26.
