Total alkalinity was not tested weekly. -Observed that alkalinity is not being recorded weekly. -Test total alkalinity at least once per week.

3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Equipment was not maintained per manufacturers specifications. -Observed weir door missing. -Replace missing weir door.

3701-31-04 (B)(6)(s) / Perimeter barrier is in good repair to prevent unauthorized entry
Gates or doors were not self-closing and lockable. -Observed interior door to pool does not completely latch and close on its own. -Repair gates or doors in order to be self-closing and lockable.

3701-31-04 (B)(6)(y) / Chemicals are properly stored/used
Critical Chemical containers were stored without labels identifying the contents. -Observed spray bottle with no label. -Label and properly store all chemical containers to avoid a chemical reaction or unnecessary exposure.

3701-31-04 (C)(2) / pH is within 7.2 to 7.8
pH was outside the range of 7.2 to 7.8. -Observed pH was below the acceptable range at <6.8 -Treat the pool water so that the pH is within the acceptable range.

3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Total alkalinity was less than 60 ppm. -Observed the alkalinity was below 60 ppm at 30ppm. -Raise the total alkalinity to at least 60 ppm.

3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch for a shut off valve was not provided or the unit was not tested monthly. - Observed the ACC unit not being recorded as tested monthly. -Provide records indicating that the unit is being tested monthly.

Test kit reagents were not fresh or stored properly. -Observed expired reagents -Obtain fresh reagents and store them properly.

Comments
-Observed the tested pH value of pool water did not match the pH value recorded on the ACC. Strongly recommend promptly having the ACC serviced or calibrated.
-Ensure the water chemistry is being tested at the required frequencies. Compare the measured values to the ACC to ensure they match.
-The Certificate of Compliance for the main drain covers is available in the equipment room.
-Discussed the requirements for documenting accidents.
-Due to the number of violations and two critical violations, a re-inspection will be conducted in approximately 1 week.
