| Equipment requiring ODH plan approval has been replaced since the last inventory report dated 3-27-17 without submittal of an ERN. The disinfection chemical feeder AutoPolit RC 35/22 has been replaced with a Stenner 45M5. The type of system was changed from a salt/chlorine generator cell to a sodium hypochlorite feeder pump. An automatic chemcial controller AutoPolit 75003 has been replaced with a Hayward HCC 2000. TO AVOID ENFORCEMENT ACTION SUBMIT AN EQUIPMENT REPLACEMENT NOTIFICATION TO THE OHIO DEPARTMENT OF HEALTH WITHIN ONE WEEK. |
