Backflow prevention devices were not maintained to function properly. There are 2 hose bibs in the filter room that are not backflow protected.

Backwash lines and lines from drainage sumps did not have an air gap prior to discharge. Only a air break now exists.
Open the lines that discharge into the backwash or drainage sumps so that there is an air gap and the lines can drain freely. PROVIDE AN AIR GAP FOR THE FILTER BACKWASH LINE. Correct By: 09-Jul-2017

Throttle valve was not properly maintained/adjusted. The throttle valve was not tagged with the max and min flow.
TAG THE THROTTLE VALVE WITH THE MAXIMUM ALLOWABLE FLOW AND THE MINIMUM REQUIRED FLOW OR MARK THE PIPE BY THE FLOW METER WITH THE MAX AND MIN FLOW. MAXIMUM ALLOWABLE FLOW IS TO BE DETERMINED. THE MINIMUM REQUIRED FLOW IS 343 GPM. Correct By: 09-Aug-2017

Depth markers/deck warning signs were not maintained in a clearly legible/visible manner. Depth markings and "No diving" markings are more than 25 ft apart on the deck.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. PLACE ADDITIONAL DEPTH MARKINGS AND "NO DIVING" MARKINGS WHERE THEY ARE MORE THAN 25 FT APART. Correct By: 09-Aug-2017

THE MAXIMUM ALLOWABLE FLOW FOR THE FILTERS AS MARKED ON THE DATA PLATE IS 410 GPM. THIS INFORMATION WILL BE VERIFIED WITH A CALL TO THE MANUFACTURER.

THE FLOW METER IS NOT PROPERLY LOCATED. THE FLOW METER IS 17 INCHES UPSTREAM FROM THE 90 DEGREE ELBOW IN THE 6 INCH PIPE.

THE BACKFLOW PREVENTER ON THE INCOMING CITY WATER DOES NOT APPEAR TO BE BACKFLOW PROTECTED. THE ASSE 1015 LOCATION IS IN QUESTION. THE TESTING OF THE UNIT ALSO DOES NOT APPEAR TO BE CURRENT.
