Replace the missing equipment labels or have the proper equipment information on site for each piece of equipment.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met. Flow meter at 200gpm - minimum required flow is 408gpm.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow switch for Automatic Chemical Controller (ACC) was not tested monthly
Have the ACC unit tested & recorded monthly. Provide records indicating that the unit is being tested monthly.
