Repeat The licensee /authorized representative was not keeping or maintaining operational records. Daily records have not keep kept for more than 3 months except for one day a week when the contracted pool company checks water.
All facility records must be maintained and available for review upon request. KEEP DAILY OPERATIONAL RECORDS. POOL SHOULD NOT OPERATE IF RECORDS ARE NOT KEPT DAILY.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE THE FLOW RATE TO AT LEAST 57 GPM. Correct By: 08-Mar-2019

Deck surfaces had standing water or other hazards. Mold is growing on the walls and floor where water condenses alone the outside walls.
