3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Repeat Total alkalinity was less than 60 ppm.
Raise the total alkalinity to at least 60 ppm.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Repeat Flow or pressure switch was not tested monthly
Provide records indicating that the unit is being tested monthly. TEST AND RECORD THE ACC FLOW SWITCH FUNCTION.
