Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).

3701-31-04 (B)(6) / Equipment is maintained per manufacturers specifications.
Repeat Equipment was not maintained per manufacturers specifications. ACC shut off valve to the test cell is broken.
All equipment must be maintained in accordance with the manufacturers specifications. REPLACE THE BROKEN VALVE TO THE ACC TEST CELL.
3701-31-04 (B)(6)(c) / Cross connection hazards shall be prevented.

Remove all cross connection hazards from the pool area. PROVIDE AN AIR GAP ON THE FILL LINE.
3701-31-04 (B)(6)(o) / Ladders/stairs/steps are in good repair/slip resistent
Ladders/stairs/steps were not in good repair or were not slip resistant. Ladder missing step tread.
Repair or replace the ladders/stairs/steps so that they are slip resistant and in good repair. REPLACE THE MISSING TREAD ON THE LADDER STEP. Correct By: 19-Jun-2023

Repeat Gates or doors were not self-closing and self-latch.
Repair gates or doors in order to be self-closing and lockable. REPAIR THE GATE SO THAT IT SELF-LATCHES.

Repeat Perimeter barrier had gaps greater than 4 inches.
Repair the perimeter barrier so that there are not any gaps greater than 4 inches anywhere along the barrier. REPAIR THE FENCE WHERE HOLES AND GAPS ARE UNDER THE FENCE.
3701-31-04 (D)(05) / Cyanuric acid level shall not exceed 70 ppm.
Repeat Cyanuric acid level exceeded 70 ppm. CYA level is above 100 ppm
Drain the pool at least halfway and fill with clean water. Monitor the amount of cyanuric acid being used and adjust accordingly. LOWER CYA LEVEL TO BELOW 70 PPM. Correct By: 19-Jun-2023

Repeat First aid equipment was moldy.
Provide first aid equipment, including a first aid kit, at the pool at all times. REPLACE THE MOLDY FIRST AID SUPPLIES. PROVIDE GLOVES.

Our records show that the main drain suction outlet cover (AquaStar 8MF) was installed 8/5/2019. The cover has a 5 year life span and must be replaced before 8/5/2024.

CDC recommends that the chlorine level not exceed 10 ppm.
