3701-31-04 (B)(4)(c) / SVRS/APSOS/ACC test results/ service records are maintained
Repeat SVRS and ACC test results were not maintained.
The test results for these devices must be kept on site for review. TEST AND RECORD RESULTS FOR THE ACC AND THE SVRS ON THE WEEKLY LOG SHEET.

Critical Repeat Chemicals not stored properly.
Properly store all chemical containers to avoid a chemical reaction or unnecessary exposure. -MOVE ALKALINITY UP CONTAINER FROM UNDER THE LIQUID CHEMICALS. -REMOVE THE WOOD STAIN AND PAINT FROM THE PUMP ROOM. -STORE THE BAGS OF SHOCK OFF OF THE FLOOR.
