Repeat Backflow prevention devices were not maintained to function properly. An ASSE 1015 backflow preventer has been installed. This is the incorrect application for a 1015.
Have the backflow prevention device replaced so that backflow is prevented. INSTALL AN ASSE 1013 ON THE WATER FILL LINE.
3701-31-04 (B)(6)(f) / Required turnover rates are sufficient
Repeat Required turnover rates were not met.
Have the circulation system evaluated to determine why the turnover rates are not able to be met. Once the reasons have been determined have the issue repaired. If necessary, submit plans to the Ohio Department of Health to review and approve. INCREASE FLOW RATE TO AT LEAST 76 GPM.

Repeat Depth markers/deck warning signs were not maintained in a clearly legible/visible manner. The depth markings are more than 25 ft apart in the southeast corner.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. ADD ONE DEPTH MARKING IN THE SOUTHEAST CORNER.
