[?] A summary of the violations found during the inspection are listed below.

3701-31-04 (A) / Approval shall be obtained for any construction or alterations of a public swimming pool.
Repeat Equipment, at a public swimming pool, was changed/altered without obtaining approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN).
Obtain approval from the Ohio Department of Health (ODH) by submitting an Equipment Replacement Notification (ERN) before operating pool. SUBMIT AN ERN FOR THE REPLACEMENT OF THE DISINFECTION FEEDER AND THE CIRCULATION PUMP.
3701-31-04 (B)(1)(a) / Outlet covers shall be installed/secured/in compliance
Critical Repeat Outlet covers were not secured. Two screws missing in one drain cover.
Do not operate the pool when the outlet covers are not secured. Secure the covers prior to the pool being placed back into operation. REPLACE THE 2 MISSING SCREWS IN THE ONE MAIN DRAIN COVER.
3701-31-04 (B)(6)(c)(iii) / Floor areas in equipment, filter and storage rooms shall be free of standing water
There was standing water on the floor areas of the equipment room.
Remove the standing water and modify the floor so that water will flow to the drain and not be able to pool on the floor. REPAIR WATER LEAK IN THE FILTER ROOM. Correct By: 10-Aug-2022
3701-31-04 (B)(6)(i) / A means of determining rate-of-flow on all water pumps
Repeat A means of determining rate-of-flow was not found for all water pumps. The jet pump without flow meter or vacuum gauge.
Properly install a flow meter or have a pump curve available for every water pump used for the pool. INSTALL MEANS TO DETERMINE FLOW RATE FOR THE JET PUMP.
3701-31-04 (B)(6)(q) / Depth markers/deck warning signs are maintained
Repeat Depth markers/deck warning signs were not maintained in a clearly legible/visible manner.
Replace the depth markers/deck warning signs so that bathers are able to clearly see the depth of the water and that no diving is permitted where clearly indicated. PLACE 2 DEPTH MARKINGS ON RIM.
3701-31-04 (C)(2) / pH is within 7.2 to 7.8
Repeat pH was outside the range of 7.2 to 7.8.
Treat the pool water so that the pH is within the acceptable range. ADJUST pH
3701-31-04 (C)(3) / Total alkalinity is at or greater than 60 ppm
Repeat Total alkalinity was less than 60 ppm.
Raise the total alkalinity to at least 60 ppm. ADJUST ALKALINITY.
3701-31-04 (D)(7)(d) / Flow or pressure switch with a shut off valve shall be provided and tested monthly.
Critical Flow or pressure switch was not tested monthly
Have the ACC unit serviced or replaced with a unit that has a flow or pressure switch with an automatic shut off valve. Provide records indicating that the unit is being tested monthly. RECORD ACC FLOW SWITCH TEST RESULTS ON THE LOG SHEET. Correct By: 10-Aug-2022
